The clinical programs prioritize the integration of clinical research with clinical care and seek to deliver cutting edge, personalized therapies to patients with mood disorders drawing on emerging developments from the Center-supported research efforts.
Precision Mood Research Clinic
The Precision Mood Research Clinic is led by Dr. Krystal and under this clinical model, all patients with depression who come to UCSF will be selected at random to receive either personalized care based on published research or depression-care-as-usual. This will allow us to test the hypothesis that personalized care leads to significantly better outcomes than care-as-usual. We are testing our hypothesis based on evidence suggesting that there are specific subtypes of depression that are responsive to specific treatments. Personalization will be determined by lab tests, clinical assessments, and neuroimaging; outcomes will be systematically assessed in all patients who consent. Those not selected to receive personalized care will receive a comprehensive evaluation by a multi-disciplinary team of clinicians for patients. Those patients also will receive a detailed diagnostic assessment and specific set of treatment recommendations, including referral to ongoing clinical research studies such as the DBS project.

Transcranial Magnetic Stimulation (TMS) Clinic
The TMS program co-led by Drs. Krystal and Scangos provides patients with mood disorders evaluations for potential TMS treatment as well as courses of TMS therapy in cases where it is indicated. Clinical research carried out in conjunction with the TMS program focuses on attempting to build on findings emerging from the intracranial closed-loop DBS project. Currently this consists of attempting to replicate with non-invasive means (fMRI), personalized biomarkers of major depression identified via invasive methods employing intracranial EEG (iEEG) recordings.
